The easiest way to have a combined TOC is to make a page that transcludes the contents of all the subpages. See the HIG for an example of this in action. --Sj talk 17:29, 13 October 2008 (UTC)

Can you describe in a bit more detail what you want the TOC to look like and where you want it to show up? Would this just be a single page with nothing but TOC? See also what wikitravel does with its auto-collapsing TOCs that by default show the top level sections, but can be expanded... for a long TOC this is quite nice.
